Welcome to Salt Lake International Driving School (SLIDS). Our instructors are State Certified and trained through the National
Safety Council. We look forward to collaborating with you to provide you with an enjoyable and safe learning environment
that will prepare you for the challenges Utah roadways have to offer. Whether you are new to driving, new to the area, preparing
for the State Road Test or just need additional driving practice on the freeway, you have come to the right school to equip you with
safe and defensive driving skills. Please review the program rules and regulations before signing the registration form and making
payment for the services we offer. This is our legal binding contract. Thank you in advance for trusting SLIDS with your driver
training needs and we appreciate your full cooperation with the following terms and conditions.
General Rules: Salt Lake International Driving School is an LLC registered with the State of Utah, Department of Commerce,
licensed by the City of South Salt Lake to conduct business in the city, and certified by the Utah Department of Public Safety Driver
License Division as a Commercial Driver Training and Driver Testing School. Our school is owner-operated, with low overhead costs,
therefore passing all the savings on to you while providing you with exceptional Driver education experience.
SLIDS will provide students with driver education services based on individual student needs. SLIDS does not have a one-size fits
all policy. Every student driver is unique therefore the training provided will be catered to the student while exceeding the state
minimum requirements for licensing in the State of Utah. SLIDS offers Complete Driver’s Ed course, Behind-The-Wheel only
Course, Private Driving Session, Refugee focused Driver Education course and DLD approved State Skills assessment (Road Test).
All scheduled behind the wheel lessons and road tests will continue as scheduled after the 24-hour cancellation period. We reserve
the right to change or modify your scheduled appointment with a reasonable amount of advance notice as seen fit for the safe
operation of the program, and other business needs. Such changes will be communicated to you either by email or through the
phone number provided at registration via call or text. It is your responsibility to monitor your email and phone for any updates
regarding your scheduled driving or testing sessions. It is the student’s responsibility to contact the office for the status of their
scheduled behind the wheel or testing sessions should there be questionable inclement weather in the valley.
Finally, we reserve the right to cancel driving or testing sessions for reasonable cause (i.e., natural disaster, severe weather and alike).
SLIDS will provide you with as much advance notice as possible for any changes to your scheduled appointments. For changes initiated
by the student, SLIDS requires a minimum of 24-hr advance notice from you to make changes to your appointments, should the need
arise. Any changes within 24 hours of the scheduled start time will incur a non-refundable change fee of $50.00/Hr missed or
rescheduled for behind the wheel driving sessions. There is no refund for Road Test appointment changes or cancellations
within 24hrs of the scheduled time. You will forfeit the payment and appointment if you are more than 10 minutes late from
your scheduled start time.
Tuition: Unless prior arrangements are made, the student agrees to pay Salt Lake International Driving School the full tuition for the
selected program at the current school rate at the time of registration. We accept all major US and international cards,
VISA/MC/DISC/AMEX. We also accept cash and cashier’s checks. Salt Lake International Driving School does not accept
personal checks of any kind. Salt Lake International charges all applicable state and local taxes on all tuition and fees for all
programs. Student tuition is not transferable from one student to another. A student who defaults on payment plans and fails to
fulfil the agreed upon terms will be subject to collection fees as permitted by the State laws.
Refunds: Students are eligible for a full refund if the request is made within 24 hours of registration and prior to starting any
portion of the course. After this grace period a $100 service charge will be assessed in addition to any behind-the-wheel hours,
and classroom lessons completed before withdrawal. For the Online Drivers Ed course, refunds are only permitted prior to receiving the
welcome email with the login credentials. Once the email is sent to the registered email; the student has access to the school material and therefore
no refunds will be processed even if the student has not logged into the course. These are student specific logins and cannot be reversed or deactivated. For any behind the wheel training session, refunds are only permitted within the grace period mentioned above and before any
appointments are made and accepted by the students. There are no refunds for the Behind the wheels sessions once appointments are scheduled
and the student has accepted such appointment via google calendar invite. No refunds of any sort are allowed after six (6) months of
program enrollment.
No show Fees: A fee of $120.99 will be charged to the student if he or she is absent from a previously scheduled behind the
wheel driving appointment in addition to the course fee. The no show fee must be paid in full before any subsequent appointment
(s) are made. If the student misses two or more scheduled driving appointments without proper cancellation notice, the tuition will
be forfeited, and the student fails the course. All withdrawals or change requests must be made 24 hours before the start of the
scheduled session by contacting the school’s main phone number and leaving a message if unanswered or by sending an email to the
school email address provided at the time of registration. No re-scheduling or cancellation will be accepted via text messaging.
Course Time Limit: Complete Drivers Ed Course, Behind The wheels Only Course and Private Driving sessions and Online Drivers
Ed only course must be completed within twelve calendar months of registration and payment. A road test appointment must be
completed within one month of registration and payment. If any course is not completed within the time limit above, funds will
be forfeited, and enrollment will be discontinued. The student must repay the full registration fee and resume the course where
they last stopped. Students may begin the program on the same day a payment and completed registration form is received.
Students may choose to go at their own pace, so long as the program is completed within the prescribed time above.
Learners Permit: The student shall supply the school with a valid learner permit before any driving sessions are scheduled.
The permit must also remain valid throughout the student enrollment in any of the school programs. It is the student’s
responsibility to check all ID and testing requirements for the State of Utah before registering in any of our programs. Any
student that presents an expired learner permit, international driver license without the proper permit from the Driver’s License
Division for a scheduled driving session or road test will be asked to re-schedule the appointment at the students’ expenses for the
missed session with a rescheduling fee of $55 for test, and $120.99 for training session plus all applicable taxes and local fees.
Out of State: Out of state student trainees who have completed a portion of their state-required driver education skills hours
may complete customized behind the wheels only skills training hours to meet the minimum Utah state requirements. The
student can choose to either complete the full Behind-The-Wheels Only program (6 hours) or only complete the remaining hours
at the current private drive hourly rate. At the completion of these hours, the student will be provided a certificate showing
behind-the-wheel driving training hours completed at Salt Lake International Driving School.
Pickup/Drop Off Services: For all behind the wheel driving appointments, with prior arrangements SLIDS can provide pick up and
drop off services within the scheduled appointment time slot anywhere in Salt Lake County. All scheduled driving sessions will
begin at scheduled time from school and terminate at the school at the end of appointment slot. If pick up and drop off services
are pre-arranged, the instructor will leave the school at the start of the appointment and drop off the student 15-minutes prior to
the end of the appointment to allow sufficient time to return to school by end of scheduled appointment. The student is required to
check in and confirm a pick-up address at least two hours prior to start of appointment.
Alcohol/Drug use: Salt Lake International Driving School has ZERO tolerance for student consumption of alcohol, drugs, or any
chemical substance that may interfere with the safe operation of motor-vehicle during the session. A student suspected to be
under the influence of any alcohol or drugs, (illicit or any prescribed) impairing the student’s ability to safely operate a motor
vehicle will be asked to re-schedule their appointment for a fee of no less than $100.00 per hour missed. This will be $200.00 per
driving session missed due to drug and alcohol policy violation. It is the student’s responsibility to know the effects any
prescribed drugs will have on their driving ability before making any behind the wheels training or assessment appointment.
Smoking/ Personal Electronic Device use: Smoking and vaping is not allowed during lessons (classroom or in- car sessions).
Smoking is not permitted within thirty feet of the school vehicles or buildings. The use of personal electronic devices during
class or while training or testing in the car is strictly prohibited. Students in violation of our smoking and electronic use policy
will be given warnings as seen fit for the successful completion of the program. Any student who willfully fails to comply with our
smoking and personal electronic device use policy will be dismissed from the program with no refund.
Skills Assessment/Road Test: Congratulations on reaching this milestone. Thank you for trusting us to evaluate your safe driving
abilities. Salt Lake International Driving School is pleased to offer you a DPS approved, third party road test. A student may
choose to use their own vehicle or school owned vehicle for this practical skills test. If a student chooses to use their own
vehicle, the vehicle must be properly licensed, insured and in good mechanical condition.
A student trainee is limited to one skills test in any 24-hour period per division rules. If a student fails a skills road test at SLIDS
or any other school, a student may re-take this test with SLIDS for a fee per test after a 24-hr waiting period from the end of
the previous test. There is no limit to the number of attempts a students may make to pass a Utah Road Test. However, at the
discretion of the school staff, a student may be required to take additional driver training courses at SLIDS or at any other school
after three or more unsuccessful attempts to pass the driving test.
Remedial Training: SLIDS may require the student to provide proof of additional behind-the-wheels training before rescheduling any subsequent driving tests. Salt Lake International Driving School offers students a customized behind the wheels
training under our private drive program to help the student with establishing safe driving skills. The student will then be
instructed to get additional skills testing at a different third-party tester or at the Driver License Division office at the students’
expense should SLIDS provide any amount of behind-the-wheel training to a student after a failed road test attempt. Please note,
per Utah rule 708, Salt Lake International Driving School will not offer the skills assessment/road test to students who have
completed any amount of behind-the-wheel training hours or classroom training at our school.
Guarantees: Salt Lake International Driving School does not guarantee any student passing any part of subsequent testing after the
completion of our program (s). We will do our best to equip you with the skills necessary to be a safe driver on Utah roadways.
SLIDS does not guarantee a passing score of any road test, regardless of the number of attempts a student makes to pass the test
at SLIDS or any other school. Each test is independent and scored as a new test, therefore it is the student’s responsibility to prepare
adequately before attempting to take the Utah Skills Road Test.
Certification: As required by Utah Rule 708-2-16, upon completion of any of our training programs, you will receive a certificate
of completion in your name providing proof of your training completion. You may present this certificate with separate
documentation showing the completion of classroom training (if applicable) from another approved program to meet the driver
license division’s requirements for driver education. Please note, Driver Education training completion requires both classroom
and a minimum of 6-hours behind the wheels in car session to issue a certificate of completion. If only one of the two portions
is completed at our school, SLIDS will not issue a certificate of completion for the one portion, however your training records
will be shared with the Utah Driver License Division, Driver Education Unit for licensing purposes.
